For all that cats like their space and are known to be aloof, we, as cat owners, know that - sometimes - out cats like to be the very center of attention. And those cats will do everything that they need to do to get it. We all, of course, know that cats love breaking into zoom calls for no particular reason. We have seen a cat crash into a lawyer's swearing in ceremony. We have seen a cat walk a literal cat walk and steal the whole show. Cats only pretend that they don't want attention when, in reality, cats want all the attention, they just want it on their terms.
It just so happens that cats want attention specifically when we are too busy to give it. And that's pretty much what happened in this twitter thread. Two doctors were having a conversation in the middle of the night (and no, we don't know why it had to be the middle of the night, but it was), when the cat decided that it's time to steal all the attention, and - of course - the cat got what it wanted.

Uhm yes, it would indeed be weird if you said no, because then the only other implied explanation that you, Mr. Josh Trebach, MD, decided to meow yourself for no reason in the middle of the conversation. Questionable behavior, but acceptable- under some circumstances.
Now, this is the right reaction to have. We would have done the same thing, so we can only respect this man. Being made aware that one of your colleagues has a cat is a call to ask for cat pictures. That's how life works.
