OP was given this repot as a warning for "policy violations" and "insubordination" for allegedly talking about their salary. It is illegal in the US to write someone up for discussing salary.

This was another thread we found from the OP's account. They appeared to be having other troubles with their employer before leaving. It does not seem like things were good.
