But how can one know where to draw the line? When does it stop being funny and become toxic?

It's not easy, realizing that the friendship you have with someone might not be healthy, and that you may be better off without them. Usually, this realization doesn't happen overnight. It could be after a few disappointing meetings, a joke that has gone too far, or perhaps you simply understand that you feel better when they are not around. Either way, everyone has had to leave a friend or two behind.
